PARTICIPANT FORMS
All 2012 AF SFFP participants should carefully read through the 2012 AF SFFP Terms and Conditions document. With the exception of the Faculty and Graduate Student Acceptance Forms and the Direct Deposit Authorization Agreement Form (see below), this document contains all the remaining forms which pertain to the fellowship.
BEFORE the fellowship commences
Forms to be submitted to sffp@asee.org:
- Faculty Acceptance Form (faculty only)
- Graduate Student Acceptance Form (graduate students only)
- Acceptance of Terms and Conditions Form
- Temporary Moving Allowance Request Form
Form to be submitted via postal mail (see address at the bottom of this page):
- Direct Deposit Authorization Agreement Form (with attached voided check)
AFTER the fellowship has begun
Forms to be submitted to sffp@asee.org:
- Official Start Date Form
- Relocation Allowance Reimbursement Request Form (with copies of receipts, boarding passes, luggage fees, maps indicating proof of driven mileage, etc.)
AFTER the fellowship has been completed
Forms to be submitted to sffp@asee.org:
- Final Report Abstract and Cover Sheet Form
- Patent and Inventions Disclosure Form
An Evaluation Questionnaire will also be available at the end of your fellowship.
